what thank you gift for a doctor?

2 years ago Mrs Jaymon had a surgery that basically changed her life. Arch reconstruction on her foot. After recovery, now that her foot no longer hurt with every step, she went on a massive journey of self improvement via exercise. She wants to send the surgeon a thank you gift, and maybe a before/after photo showing how much the surgery meant to her.

What do ya'll think? is this sort of thing appropriate/ and what kind of gift ?

As a doctor, I can tell you that gifts are nice, but cards with detailed happy stories and gratitude are magnitudes better. It will be even more amplified because the event was a few years ago and it made so much of an impact that you reached out.
